RELEASE OF LIABILITY, ASSUMPTION OF RISK AND MEDICAL AUTHORIZATION Capistrano United tryouts I am the parent or legal guardian of the child named in this registration. In exchange for my child being allowed to take part in the Capistrano United tryouts, I agree to the following. 1. Assumption of risk. Soccer is a physical contact sport. Taking part in the tryouts involves risks, including falls, collisions, sprains, fractures, concussions, heat-related illness and flare-ups of existing conditions such as asthma or allergies, and in rare cases serious injury. I knowingly accept these risks on behalf of my child and myself. 2. Health and fitness. I confirm that my child is physically able to take part. Before the session starts, I will tell the coaches about any medical condition, allergy or medication they should know about (for example, asthma and an inhaler), and I will make sure my child brings any medication they may need. 3. Release. To the fullest extent allowed by law, I release, waive and agree not to sue Capistrano United, its owners, directors, coaches, staff and volunteers, and the owners and operators of the fields used (together, "the Club"), for any injury, illness, loss or damage to my child or to me arising from the tryouts, including claims based on the Club's ordinary negligence. This release does not cover gross negligence or intentional misconduct. 4. Medical authorization. If my child needs medical attention and I cannot be reached right away, I authorize the Club to give first aid, call emergency services and arrange emergency medical treatment. I am responsible for any medical costs. 5. Supervision and conduct. My child must follow the coaches' instructions, and the Club may ask a child to stop taking part for safety reasons. I am responsible for my child before and after their session. 6. General. This agreement is governed by the laws of the State of California. If any part of it is found unenforceable, the rest still applies. The name I type below is my electronic signature and has the same effect as a handwritten signature. I have read this release, I understand it, and I sign it voluntarily.
